Medication adherence with clients with schizophrenia - Essay Example The prevalence of medication non-adherence has been reported to be almost half of the total population of patients with schizophrenia (Tabor & Lopez, 2004). The prevalence could even be higher considering that the figures do not count patient with schizophrenia who decline treatment or quit routine check-up. Hence, a frequent cause of treatment failure is medication non-adherence, and the most widespread kind of non-adherence is inadequate use of these drugs. The consequences of non-adherence involve disruption of work activities and home life, re-hospitalization, and recurrence of psychotic symptoms. Monitoring adherence to medications and evaluating side effects is an important medical task when caring for patients with schizophrenia (Higashi et al., 2013). This essay reviews the literature on medication adherence and non-adherence in patients with schizophrenia. Determinants of non-adherence to medication can be classified into four, namely, patient-based aspects (e.g. gender, age , income, education), health-based aspects (e.g. substance abuse, duration of ailment, symptomatology), aspects related to antipsychotic medication, and socio-environmental aspects (e.g. social support, relationship between healthcare provider and patient). The literature review of Higashi and colleagues (2013) explains that there are numerous studies that investigated the correlation between socio-demographic factors (e.g. gender, age, education) and medication non-adherence but discovered no consistent relationship. There are findings revealing that adolescents are less likely to adhere to medication than their older counterparts. Several studies have discovered no relationship between gender and non-adherence, but some researchers, like Tunnicliffe and associates (1992 as cited in Higashi et al., 2013), discovered that females have a greater tendency than males to adhere to anti-psychotic medication. It has also been reported that certain ethnic minority groups, such as African A mericans, tend to be noncompliant. But there are other researchers, such as Owen and colleagues (1996 as cited in Higashi et al., 2013), who found no correlation between ethnicity and medication non-adherence. The researchers conclude that almost all studies report little or no correlation between socio-demographic factors and medication adherence. Lack of insight is a major attribute of individuals with schizophrenia. It is easy to understand why individuals with poor awareness have showed a greater risk for noncompliance to medication. There is no definite explanation of awareness in psychotherapy, but usually, it means understanding of oneââ¬â¢s sickness and its outcomes or effects (Kozuki & Froelicher, 2003). Rusch and Corrigan (2002) enumerate four features of awareness: cognitive features, adherence based features, symptom based features, and temporal features. Lack of awareness has major therapeutic, clinical, and social significance. Nevertheless, its repercussions on med ication adherence are most vital in the recovery period of schizophrenia. The patientââ¬â¢s extent of awareness of the disorder is directly associated with his/her attitudes toward or beliefs about the treatment. Frost shows a similar message in his poem, although he gives honor to the one who is the object of that poem. Frost sets up a story within his poem of a boy who has a terrible accident. He shows how quickly everything can turn from being normal towards a terrible event. The narrative is the story of a saw that cuts t he boyââ¬â¢s hand, his approximate age indicated by the lines ââ¬Å"Then the boy saw all - /Since he was old enough to know, big boy/ Doing a man's work, though a child at heart ââ¬âââ¬Å". In this poem, the individual is valued. The narrator of the poem shows sentiment towards the boy, his words ââ¬Å"Call it a day, I wish they might have said/ To please the boy by giving him the half hour/ That a boy counts so much when saved from workâ⬠suggesting that he wanted him to have a good experience. The Power of Truth Essay Novelist Tim Oââ¬â¢Brian once said ââ¬Å"A lie, sometimes, can be truer than the truth, which is why fiction gets written. â⬠In his novel The Things They Carried, Oââ¬â¢Brian argues that ââ¬Å"story-truth is sometimes truer than happening-truthâ⬠(171). Oââ¬â¢Brian opposes the idea of absolute truth, and believes that all truths are subject to change. He believes that a fictional story can be more true than an actual event. However, a story, no matter what story, cannot be more true than an actual event; a ââ¬Å"story versionâ⬠of an event is merely a shadow of that eventââ¬âa make-up-caked, dramatized, Hollywood shadow. It is impossible for ââ¬Å"story-truthâ⬠to be truer than ââ¬Å"happening-truth. â⬠In The Things They Carried Oââ¬â¢Brian creates a daughter, Kathleen, for the fictional version of himself. Oââ¬â¢Brian, however, does not have a daughter in real life. While it may be a ââ¬Å"story-truthâ⬠that Tim Oââ¬â¢Brian had a daughter, it will never be a ââ¬Å"happening-truth. â⬠A made-up daughter will never be more real than Oââ¬â¢Brianââ¬â¢s actual son. In the novel, Oââ¬â¢Brian ââ¬Å"return[s] with [his] daughter to Vietnamâ⬠where he buries a belonging of a fellow soldier (173). This story can never be more true than an actual event solely because of the involvement of a fictional character. A man can tell a story about his last trip to the grocery store, and most people would count it as true. However, if the man told a story about going to the grocery store with a magic genie, it would be assumed that he is lying, even if every other facet of his story was believable. This is no different than Oââ¬â¢Brian weaving a fictional character in to his novel. The ââ¬Å"story-truthâ⬠of Kathleen can never be more true than ââ¬Å"happening-truth. â⬠Furthermore, in a crime investigation, it is illegal to interfere with evidence involved with a case. Any disruption or corruption of evidence is termed ââ¬Å"falsifying evidence. â⬠This is a serious offence because it could seriously disturb the process of solving a crime. Devoid of sound evidence, it is difficult to determine how a crime was committed. Evidence is an example of ââ¬Å"happening-truth;â⬠it conveys without any bias, exactly what happened at the scene of a crime, or at the events leading up to a crime. Investigators cannot solve a crime based solely off of ââ¬Å"story-truth,â⬠such as the testimonies of witnesses, because of the numerous individual accounts of a crime which are subject to dishonesty, human error, and prejudice. Therefore, investigators rely on ââ¬Å"happening-truthâ⬠because it is trustworthy, whereas ââ¬Å"story-truthâ⬠is inconsistent. Moreover, the popular film The Titanic is bursting with examples of ââ¬Å"happening-truthâ⬠and ââ¬Å"story-truth. â⬠Director, James Cameron, designed many of these ââ¬Å"story-truthsâ⬠to provide an increased dramatic tone in the film. He created a grand staircase on the ship while in reality this staircase was so small passengers had to go up and down single file. A Pablo Picasso painting is featured in the film, although Picasso was not popular during the time of the Titanic, and his work was not widely distributed. The diamond necklace in the film, titled ââ¬Å"The Heart of the Ocean,â⬠never existed, much like the love story between invented passengers Rose Bukater and James Dawson. These creations served Cameronââ¬â¢s purpose to paint a theatrical telling of the event, but the movie was just thatââ¬âtheatrical. Although these ââ¬Å"story-truthsâ⬠indicate the sinking of the Titanic was a romantic event, it is the ââ¬Å"happening-truthsâ⬠that send chills through the viewersââ¬â¢ astonished bodies. The most powerful aspect of the film The Titanic is not the love story. It is not the grand staircase, or the luxurious dining room. It is not the interaction between characters, or the extravagant ââ¬Å"Heart of the Ocean. â⬠The most powerful aspect of The Titanic is that it actually happened. At the end of the film, there were 1,502 dead bodies sinking and in the freezing salt water of the Atlantic. No ââ¬Å"story-truth,â⬠no matter how elaborate, could create a greater tragedy than a ââ¬Å"happening-truth. â⬠Why not? Since ââ¬Å"story-truthsâ⬠are not as true as ââ¬Å"happening-truths. â⬠Therefore, the notion that a story could ring truer than an actual event is in the wrong. A lie cannot reveal a deeper truth; only truth can uncover truth. Stories provide an escape for the imagination. They allow the reader to imagine a different reality and to glimpse into an alternate world. However, stories are only stories. They are not real. The only way the reader can understand a true reality is to read a true account of that reality. Although this account may be less dramatic than the ââ¬Å"story versionâ⬠of the reality, it is in fact the reality. It is the truest truth; the ââ¬Å"happening-truth. â⬠Works Cited OBrien, Tim.
